Dry Needling & IMS

At South Okanagan Physiotherapy, our physiotherapists use dry needling and Intramuscular Stimulation (IMS) to release tight, painful muscles and calm irritated nerves. We offer dry needling across our Osoyoos, Oliver, Grand Forks, and Penticton clinics, always as part of a full physiotherapy plan that gets you moving and keeps you moving.

What is Dry Needling & IMS?

Dry needling uses very fine, single-use, sterile needles to target trigger points: tight, tender knots in a muscle that can cause local pain or pain that travels to other areas. It is called “dry” because nothing is injected. Placing a needle into a trigger point often produces a brief twitch or cramp, after which the muscle relaxes, blood flow improves, and pain eases.

Intramuscular Stimulation (IMS) is a type of dry needling based on the idea that chronic muscle tightness is often driven by irritated nerves near the spine. Along with needling the tight bands of muscle, your physiotherapist may needle the muscles beside the spine at the nerve level involved, treating both the source and the symptoms.

Unlike traditional acupuncture, which is based on meridians and the flow of Qi, dry needling and IMS are based on a Western, anatomical assessment of your muscles, nerves, and movement. Looking for traditional acupuncture? See our Acupuncture & IMS and Traditional Chinese Medicine & Acupuncture pages.

Conditions Dry Needling & IMS Can Help With

Your physiotherapist will tell you whether needling suits your condition. It is commonly used for:

  • Neck and Back Pain: Chronic or recurring neck pain, lower back pain, and muscle spasms.
  • Headaches: Tension-type headaches linked to tight neck and shoulder muscles.
  • Shoulder Pain: Rotator cuff irritation, shoulder impingement, and upper-trapezius tightness.
  • Elbow Pain: Tennis elbow, golfer’s elbow, and forearm overuse.
  • Hip, Knee and Leg Pain: Hip and buttock pain, sciatica-type symptoms, IT band tightness, and calf strains.
  • Tendinopathy: Ongoing tendon pain that hasn’t settled with rest alone.
  • Sports and Workplace Injuries: Muscle strains and overuse injuries, including ICBC and WorkSafeBC claims.
  • Chronic Muscle Tightness: Long-standing tension that keeps coming back despite stretching or massage.

Is Dry Needling Safe?

Yes, when it is done by a trained physiotherapist. We use single-use, sterile needles and do a full health history first. Needling may not be right for you if:

  • You are pregnant
  • You have a compromised immune system, a fever, or a local infection or wound in the area
  • You had surgery in the area within the last 3–6 months, or lymph node removal in that limb
  • You take blood thinners or have haemophilia (you may still qualify with written approval from your doctor)
  • You have a strong fear of needles. Let us know, and we’ll use other techniques.

Do You Have an ICBC or WorkSafe Injury Claim?

Dry needling and IMS can be part of your physiotherapy treatment for ICBC and WorkSafeBC injuries. We handle the paperwork so you can focus on recovering, with no user fees or out-of-pocket costs for approved claims.

  • ICBC Clients: Pre-approved for 25 physiotherapy sessions, 25 chiropractic sessions, and 12 additional therapy sessions (massage therapy, kinesiology, or acupuncture) with no out of pocket expenses or user fees.
  • WorkSafeBC clients: If you have an approved claim, you are eligible for 12 physiotherapy sessions within your initial approval, ensuring you receive the care you need to return to work safely with no out of pocket expenses.
  • Additional Sessions Available: If further treatment is necessary, we can assist in requesting extended coverage for continued rehabilitation.

  1. Initial Consultation: We discuss your symptoms, goals, and medical history, and screen for anything that rules out needling.
  2. Thorough Assessment: Your physiotherapist checks your posture, movement, muscles, and nerves to find what is driving your pain.
  3. Needling Treatment: Fine needles are placed into the target muscles. You may feel a brief twitch, cramp, or deep ache that eases quickly.
  4. Active Rehab: We follow up with hands-on therapy and exercises to lock in the new range of motion.
  5. Aftercare: Mild soreness for 24–48 hours is normal. Stay hydrated, keep moving gently, and use heat if you are sore.

Does dry needling hurt?

Most people feel little as the needle goes in. When a needle reaches a tight muscle you may feel a short twitch or cramp, which usually means the muscle is releasing. Some soreness for a day or two afterwards is normal.

What is the difference between dry needling and acupuncture?

Both use the same fine needles. Acupuncture is based on Traditional Chinese Medicine and meridians. Dry needling and IMS are based on a Western assessment of muscles and nerves and target trigger points and irritated nerve roots.

How many sessions will I need?

It depends on your condition. Many people notice a change within the first few sessions. Your physiotherapist will recommend a plan after your assessment.

Is dry needling covered by my extended health benefits?

When it is done by a physiotherapist, dry needling and IMS are usually billed as physiotherapy. We direct bill most providers and can help you check your coverage.
